چکیده
A 67-year-old man with chronic lymphatic leukemia presented with a 1-month history of subacute right hemiparesis with right central facial palsy and right extensor plantar response, but without sensory deficits. MRI showed hyperintense lesions along the pyramidal tracts on T2-weighted and FLAIR images (figure). CSF PCR revealed JC virus infection leading to the diagnosis multifocal leukoencephalopathy (PML), which typically occurs in immunocompromised patients, due to reactivation of latent JC virus infection. PML lesions are hyperintense on T2-weighted and FLAIR MRI, nonenhancing, not space occupying, and predominantly subcortically located involving the arcuate (U) fibers. Posterior fossa and basal ganglia lesions are less frequent (15 to 30% of patients) and rarely occur isolated. A faint, peripheral enhancement is seen in up to 10% of patients.1 Thus far, PML with a predominant involvement of the pyramidal tract has been described only in one (HIV-positive) patient.2

Unusual MR Feature Presenting Discrete Involvement of Pyramidal Tract in Progressive Multifocal Leukoencephalopathy: A Case Report
Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease caused by reactivation of JC virus in immunocompromised patients. To date, PML with discrete involvement of the pyramidal tract has been described in only two patients. This report describes an additional case with PML showing discrete involvement of the pyramidal tract on T2-weighted images and FLAIR images.

Polyomaviruses may cause human disease, particularly in immunocompromised hosts. JCV, one of the members of polyomaviridae family, is the causative agent of the neurological disease progressive multifocal leukoencephalopathy (PML), which occurs mostly in immunocompromised patients. Progressive Multifocal leukoencephalopathy (PML) is a progressive demyelinating disorder of the central nervous sy...

A 39-year-old male presented with slurred speech, left hand weakness, gait unsteadiness and headaches over a period of 4 weeks. A brain MRI revealed a right motor strip lesion with Wallerian degeneration along the pyramidal tract plus brain stem and cerebellar FLAIR changes without enhancement ( fig. 1 ). Laboratory findings included a CD4 count of 56, HIV viral load of 2,597 and a positive CSF...
متن کاملA patient presenting with features of progressive multifocal leucoencephalopathy in HIV/AIDS.
Progressive multifocal leucoencephalopathy (PML) is associated with infection of oligodendrocytes by the papovavirus JC (the initials of the patient whose tissue was used to isolate the virus; not Jakob-Creutzfeldt disease). This leads to demyelination of the white matter. It is characterised by dementia, dysphasia, pyramidal tract involvement and persistently normal CSF analysis. The course is...
